Service Description

This subscription will give you monthly access to appointments for Urgent care and Wellness Care visits in-person or online. These visits will be for prescription refills and or evaluation and treatment of acute medical issues. Primary lab work is included. See exclusions.

Cancellation Policy

We understand that plans may change. Please notify us at least 24 hours in advance if you need to cancel or reschedule your appointment. Late cancellations or no-shows may be subject to a cancellation fee. Thank you for your understanding and cooperation.
